Acne is a common skin disease characterised by elevated sebum
production and inflammation of the sebaceous glands. It has been
previously shown that non-psychotropic phytocannabinoids exert complex
anti-acne effects by normalising ‘pro-acne agents’-induced excessive
sebaceous lipid production, reducing proliferation and alleviating
inflammation in human sebocytes. Based on their remarkable
anti-inflammatory actions, phytocannabinoids could be efficient, yet
safe novel tools in the management of cutaneous inflammations.(5)

Sebaceous glands are holocrine glands found over the entire
surface of the body except the palms, soles and dorsum of the feet. They
are the largest and mostly concentrated in the face and scalp, where
they are the sites of the origin of acne. The normal function of
sebaceous glands is to produce and secrete sebum. The sebaceous glands
further transport antioxidants in and on the skin, and exhibit a natural
light protective activity. They possess an innate antibacterial
activity and have a pro- and anti-inflammatory function.(6)

It has been clinically observed that cannabinoid abuse can be
accompanied by acne. This already highlights how cannabinoid signalling
may influence human sebocyte biology. Indeed, expression of CB1 (in the differentiated, central cells) and CB2 (predominantly in the basal, non-differentiated sebocytes) receptors has been well-documented in human skin.

In a small, single-blind, split-face study, a cream containing 3% Cannabis
seed extract was applied twice daily to the cheeks of patients for 12
weeks. The treatment was found to be efficient in reducing sebum
production and erythema compared to the vehicle treated side.(7)

Cannabidiol (CBD) is a chemical in the Cannabis sativa plant, also known
as cannabis or hemp. One specific form of CBD is approved as a drug in
the U.S. for seizures.

Over 80 chemicals, known as cannabinoids, have been found in the
Cannabis sativa plant. Delta-9-tetrahydrocannabinol (THC) is the most
famous ingredient in cannabis. But CBD is obtained from hemp, a form of
the Cannabis sativa plant that only contains small amounts of THC. CBD
seems to have effects on some chemicals in the brain, but these are
different than the effects of THC.

A prescription form of CBD is used for seizure disorder (epilepsy). CBD
is also used for anxiety, pain, a muscle disorder called dystonia,
Parkinson disease, Crohn disease, and many other conditions, but there
is no good scientific evidence to support these uses.

Laws passed in 2018 made it legal to sell hemp and hemp products in the
US. But that doesn't mean that all CBD products made from hemp are
legal. Since CBD is an approved prescription drug, it can't be legally
included in foods or dietary supplements. CBD can only be included in
"cosmetic" products. But there are still CBD products on the market that
are labeled as dietary supplements. The amount of CBD contained in
these products is not always the same as what is stated on the label. (8)

These products are permitted to contain up to 0.3% THC. Products that contain more than 0.3% THC are considered to be marijuana and must be sold from a licensed marijuana store. Any CBD product that is made from industrial hemp that doesn't exceed the federal limit of 0.3% THC can be purchased over the counter with no age restrictions. While 0.3% THC in smokable products is probably not enough THC to cause impairment, 0.3% THC in edible products, such as tinctures, gummies, or chocolates, can be a very large dose. Some products that are labeled and sold as “hemp” products contain substances like “delta-8 THC,” “THCP,” or “HHC.” These substances can cause impairment similar to or greater than the THC found in marijuana. CBN, a substance that is sometimes found in many CBD products, can cause drowsiness. Additionally, some hemp-derived products may not be produced in compliance with Colorado regulations and may not be accurately labeled. (9)

Based on the review conducted, there is sufficient clinical and preclinical evidence of CBD in pain treatment, so CBD could be an effective and safe treatment in reducing pain due to its analgesic and anti-inflammatory properties. These effects appear to be primarily mediated by the activation of TRPV-1, 5HT-1A, and CB1, with emerging therapeutic relevance in the management of osteoarthritis and chronic pain.(11)

New evidence has suggested that in humans, CBD intake could improve
satellite cell differentiation in muscles, improving muscle recovery (e.g., muscle damage attenuation) and performance (e.g., strength) (Schouten et al., 2022).
Also, recent findings demonstrate modest yet meaningful effects on muscle damage and recovery (reduction in creatine kinase and myoglobin) within a 72-h after 60 mg of CBD supplementation (Isenmann et al., 2021).
